Diabetes Prevention Program
Our MRx Navigate Diabetes Prevention Program is designed to help people with prediabetes reduce or prevent their risk of developing Type 2 diabetes. This lifestyle change program focuses on healthy eating, physical activity and weight management. The best part is that it's absolutely free for members and dependents covered by EMI health insurance!
Our program is part of the CDC-led National Diabetes Prevention Program and people who participate can cut their risk of developing Type 2 diabetes by 58%.
During this 12 month program, you will work with a dedicated lifestyle coach who will help you learn new skills, set and meet goals, and stay motivated! You will also be part of a support group where you can share ideas, celebrate successes and help each other overcome obstacles.

Program Eligibility Requirements:

Must be covered by EMI health insurance
-
Can not be diagnosed with Type 1 or Type 2 diabetes
-
Must be at least 18 years or older
-
Must have a BMI greater than 24
-
Not be pregnant
-
One or more of the following:
-
Fasting glucose of 100-125 mg/dl
-
Plasma glucose measured 2 hours after a 75 gm glucose load of 140 – 199 mg/dl
-
A1C of 5.7 – 6.4

Clinically diagnosed with gestational diabetes mellitus (GDM) during a pervious pregnancy (may be self reported)
-
A health care professional may refer potential participants to the program, but a referral is not required for participation in the CDC-recognized programs
-
OR a positive screening for prediabetes based on the CDC Prediabetes Screening Test or a screening result indicating high risk for type 2 diabetes on the hard copy or electronic version of the American Diabetes Association Type 2 Diabetes Risk Test.
